If the applicant is Under 18 or is an adult at risk this information must be agreed by a parent /carer.

• By completing the Spond sign up form, I agree to the child / adult at risk taking part in the activities of the club and I understand that I will be kept informed of these activities by the Spond App.
• I understand it is my responsibility as a parent / carer to send the medication with the child /adult at risk with their name clearly labelled on it. The person responsible for the activity will look after the medication whilst the child /adult at risk is engaged in activities. 
• I am familiar with and agree to support the child/adult member and parent / carer with RLSS UK Safeguarding Policy and its Codes of Conduct.
• I understand in the event of injury or illness all reasonable steps will be taken to contact either next of kin or the alternative contacts using the details provided via Spond, and to deal with that injury/illness appropriately.
• It may be essential on other occasions for the adult responsible for the activity e.g. coach, instructor, trainer, administrator, club official, event organiser accompanying your son/ daughter/adult at risk you are responsible for to have the necessary authority to obtain any urgent treatment which may be required whilst at Club event, representative competition or training.

I, as the parent/ carer or person responsible for the named child/ adult at risk on the Spond sign up form hereby give permission for the coach, instructor, trainer, administrator, club official, event organiser or any person responsible for any activity to give the immediately necessary authority on my behalf for any medical or surgical treatment recommended by competent medical authorities, where it would be contrary to my son/daughter’s interest or the adult at risk in the doctor’s medical opinion, for any delay to be incurred by seeking my personal consent.   “I have read and understood the RLSS UK Code of Conduct (“Code”) and as such agree to fully recognise and adhere to the principles and responsibilities embodied in the Code.”

Relevant to all applicants over 18 years old

• By submitting the completed Spond sign form, I agree to participate in the Club’s activities and events. I also understand that I will be kept informed of these activities via the Spond App.
• I am familiar with and agree to support the RLSS UK Safeguarding Policy and Codes of conduct.
• I understand in the event of injury or illness all reasonable steps will be taken to contact my emergency contacts using the details provided via Spond, and to deal with that injury/illness appropriately.

“I have read and understood the RLSS UK Code of Ethics and Conduct (“Code”) and as such agree to fully recognise and adhere to the principles and responsibilities embodied in the Code.”

• Club membership will deemed to have started upon the set up of the monthly standing order as requested by the club
• If the standing order is declined or cancelled, the club reserves the right to withdraw membership and remove all details from Spond. A new application might be required if the past member wishes to rejoin.
• The Standing order for membership/fees will be due on the 1st of each month for that month.
• No refunds will be issued for sessions not attended, or if membership is cancelled by the member or Parent/Guardian.
• Members with a current Standing Order or other appropriate current membership category, will be eligible to stand for election to the Management Committee, vote at the AGM, or other meetings such as EGMs, etc. The fees will be constantly reviewed, with the aim of keeping them the same for as long as possible, but are subject to change, with suitable notice, if required.
• In the event of a member not being able to attend for a period due to circumstance such as injury, then if the club have been notified, membership can continue with an alternative payment fee until the return to club activities. This will be agreed with the Committee.
• Any costs associated with taking awards or qualifications will require additional fees, and these will be notified to the members in advance of payment request.
• Payments are not currently taken via Spond, but this might change in the future, or apply to specific items/activities.
